To enhance community safety through enforcement of court orders while striving to empower individuals and change lives.

IT IS THE ORDER OF THE COURT THAT YOU WILL COMPLY WITH THE FOLLOWING CONDITIONS OF PROBATION:

1. I will not commit a new criminal offense.
2. I will immediately report any new criminal charge to the probation department.
3. I will not leave the State of Ohio without prior permission from the probation department.
4. I will not use alcohol or controlled substances unless prescribed to me by a licensed healthcare provider for valid medical reasons.
5. I will submit to drug and/or alcohol testing when requested by the probation department.
6. I will inform my probation officer of my address at each scheduled appointment and report any changes in address or phone number immediately.
7. I will maintain contact with my probation officer as directed.
8. I will not carry, use or possess any weapons or ammunition.
9. I will pay a $20.00 monthly probation supervision fee, unless otherwise directed by the Judge.
10. I will comply with any special conditions ordered by the Judge.
11. I waive my Fourth Amendment rights to search and seizure by my probation officer. This includes searches of my person, residence, vehicles, electronic devices, and any property under my control. Searches may be conducted by a probation officer and/or law enforcement if directed by probation.

I have read (or had read to me) the above conditions. I understand and agree to follow them. I understand that violating any condition may result in arrest, a violation hearing, and possible jail time.
